The patient: Caroline is a 40 year-old female who recently gave
birth to her second son, Michael. Her advanced age placed Caroline
into a high-risk category for having a child with birth defects. An
ultrasound performed during her eighteenth week of pregnancy was
atypical, showing absent eyes, additional fingers and toes and
abnormalities across many internal organs, Caroline elected to not
undergo follow-up diagnostic test like amniocentesis. Michael was
born with several serious birth defects and died within a week of
birth. The process: In order to determine if a chromosomal
abnormality was present, blood taken just after Michael's birth was
used to culture the white blood cells, arresting them in metaphase
of mitosis. The cells nuclei are broken open and giemsa dye is used
to stain the chromosomes. The stained chromosomes are visualized
through a light microscope and digitally photographed. As a
cytogeneticist, it is your job to use the graphical representation
of the stained chromosomes (called ideograms) to create Michael's
karyotype and provide possible insight into the cause of his
clinical symptoms.
