Ch. 4 activity, ADPIE related to pharmacology Sally Sue has been experiencing a headache "off and on" for the last few weeks. She is 29 years old and lives with a roommate in an apartment. She comes to the clinic for assessment by the healthcare provider (HCP). Her friend, and roommate, has accompanied her. The friend states she has noticed that since she has been taking "so much" Advil (ibuprofen) she has also complained of her stomach hurting and bloating. Sally Sue states she has not been eating "very well" since having the headaches. Vital signs are: B/P 122/64, pulse 78, respirations 20, Temp 97.6. She rates her current pain as a "0" on 0-10 scale. The HCP has instructed Sally Sue to limit her ibuprofen administration to two 200mg (400mg) tabs every 8 hours by mouth only as needed. The HCP also instructs Sally Sue to consume adequate fluids such as water and to eat before taking any ibuprofen. The HCP orders a neurological consult. The HCP also orders omeprazole (Prilosec) 20mg every AM 30 minutes before any other food or drink for 30 days. Based on this information let's walk through the nursing process. Assessment: I will assist with the diagnosis part as the RN. Her diagnosis will be: Knowledge deficit related to medication regimen as evidenced by administering medication without food or fluid and experiencing adverse side effects.
