The nurse Susan collected blood for blood urea nitrogen (BUN)
and creatinine tests on a patient, Bruce Jones in bed 201 at 9:30
am , and sent the specimen to the lab. At 10:30 am, Susan
calls the lab and states she is not able to find Bruce Jones's
results in the computer. The technologist, Frank, tells Susan
he does not have a specimen with that name on it; however, he did
run a BUN and creatine on a patient named Maggie Smith in bed 202
drawn at 9:30 am by Susan. Susan says there could not have
been an error because she labeled the specimen with the only label
she found on the barcoded label printer at the time.
