Assign names, styles, and locking to each content control (so that the content controls cannot be deleted). Draw a rectangle around the data entry area of the form and format the rectangle appropriately. Add a page color. Be sure to change the page size and margins, and adjust spacing as necessary above and below paragraphs, so that the entire form fits in the document window. When you are finished creating the form, protect it so that editing is restricted to filling in the form and then save it as a template with the file name, SC_WD_10_StarlightSatisfactionSurvey.
