Mr. I is admitted for an exacerbation of choronic back pain and
severe abdominal pain. Mr. I weights 100 kilograms (kg). the nurse
notifies the hospitalist about Mr. I's acetaminophen intake which
is greater than 4000 milligrams (mg) in a 24-hour period. The
hospitalist orders an acetaminophen level. Mr. I has a toxic level
of acetaminophen. the hospitalist orders N-acetylcysteine IV, 300
milligrams per kilogram (mg/kg) to be infused over 21 hours.
N-acetylcysteine is available 200 milligrams per milliliter (mg/mL)
in a 20 milliliter (mL) vial. the infusion instructions are:
initial infusion: 150 mg per kg over first 60 minutes, diluted
in 200 milliliters (mL) of DSW.
second infusion: 50 mg kg over next 4 hours, diluted in 500
milliliters (mL) of DSW.
Third infusion: 100 mg per kg over the next 16 hours, diluted in
1000 milliliters (mL) of DSW.
