A Registered Nurse (RN) on a general medical-surgical unit in a
regional hospital is working with an unlicensed assistive personnel
(UAP)/nursing assistant whom she has known and worked with for the
past 6 months. Today, she delegates the task of taking vital signs
for all patients on the 18-bed unit. She gives specific
instructions to take vital signs for patient Hana on her left arm
because the patient, who is now in stable condition, has had a
right radical mastectomy with lymph node removal. After the UAP
received instructions from the RN, she said she understood the
instructions and began the assignment. The Kardex outlining care
guidelines included that vital signs, including blood pressure
(BP), be taken on patient Hana's left arm. Additionally, the
instructions for the patient’s vital signs were documented on a
sign above her bed. Also, the UAP received a unit orientation
training upon hire that addressed taking vital signs with special
instructions. Furthermore, she received a satisfactory 90-day
probationary evaluation, which is consistent with hospital
policy.
The RN made her rounds to monitor the UAP and she finds patient
Hana in bed with the BP apparatus beside her left arm. The UAP was
not at the bedside.
