SCENARIO :
You are assigned to a female patient who was transferred to your unit earlier today. In the report, you learn that the patient has congested heart failure, pitting edema of the lower extremities, increasing shortness of breath and periods of confusion. Her vital signs are Temperature- 36.50C, Pulse- 67, RR-22, BP-175/100 mmHg. The patient wants to wash up, but she says that she does not have the energy like she used to and that she gets tired very easily. You assessed her and found the following: skin cool, dusky in color, lower extremities with 2+ pitting edema, lung sounds with crackles on inspiration. She is alert and oriented.
