145 Scenario 2 An 83-year old frail woman comes to her doctor's appointment alone. She is experiencing severe chest pain
Scenario 3 You are an OB nurse, and 15-year-old Melissa comes in for her first appointment. To help her feel less nervous, you chat about things going on at school, but she seems distracted and isn't following your conversation. When you ask about her weekend plans, she tells you that she is supposed to stay with her grandparents over the weekend while her parents are out of town and she doesn't want to go. Although she is clearly uncomfortable, she finally discloses that the last time she visited, her grandfather took her down to the basement, rubbed her between her legs, then put his hand on her chest over her breasts and told her she was pretty "just like [her] mom". She doesn't want her parents to know, but she just can't go stay with her grandparents. She doesn't want you to tell anyone.
Scenario 4 Mikel is a 16-year-old boy who has come in for his yearly physical. As you attempt to get vitals and recent medical history information from him, you note that he has little energy and just seems depressed to you. As you are taking his blood pressure, you notice that Mikel has slash marks on his forearms - old healing ones as well as fresh ones. You are concerned, and call his home and reach the father. You tell him that you have seen and what your concerns are, but the father replies, "He's a punk. He doesn't know how to be a man, so he does this stuff to get attention. I think he does it because his friends do it." You suggest that maybe Mikel should get an evaluation from a mental health professional, and the father replies, "No son of mine is going to go see a shrink. You must be joking."
Scenario 5 Jason is a 40-year-old male who has requested to come in for an appointment to discuss getting on some psychotropic medications. During the intake, you ask him what is happening. He reports feeling depressed, especially in his relationship with his wife LaShawn, to whom he has been married for 13 years. He shares that she often yells at him and makes him feel stupid. He reports that he found explicit pictures on her phone which were sent to another male. When Jason confronted LaShawn, she told him that it didn't happen, that he imagined the whole thing, and that he was crazy. In her anger, she smashed his phone. She later apologized and tearfully told him that she couldn't be in a relationship with someone who didn't trust her, and he should get on some drugs" to make him "less crazy and paranoid" Jason just wants to make her happy, citing "happy wife, happy life."
