a 57yo male with a 20 year history of diabetes mellitustype previously treated with glyburide 5mg po BID, metformin 1000mgpo BID, and rosiglitazone 8mg po QD. He is interested intransitioning to insulin therapy.
How should GD be started on insulin therapy?
How should the insulin be monitored and adjusted?
