Case Study - Romain The consumer is a 32 year old, single, Non-Hispanic, African American male who was referred by his primary care physician due to recurrent gastrointestinal distress without physical cause. Romain completed an upper and lower GI study, has been on antacid medications, ulcers were ruled out and a GI specialist was consulted without any positive finding. Roman reported a history of alcohol and marijuana use. He reported he has been in recovery for the last 3 years. He reported persistently and intensely that he does not feel he could have abused his body without any long term negative effects. He describes his symptoms as tightness in his chest, a gurgling sound in his stomach and a slight feeling of nausea. He stated these sensations are worse when he is in high places, have to cross bridges or is driving on highways. How would you start to conceptualize this case? What disorder(s) from the textbook might fit? What disorders would you want to rule out? What other disorders might be co morbid in this case? What else would you like to assess for and why? What additional information would you like to ask? Are his symptoms a normal reaction to a situation or something more? 2
