Eli is a 48 year-old man who has been living in the basement of his apartment building, eating in restaurants, and using a local YMCA for showers. His own apartment is so full of newspapers, magazines, and books that he could no longer get in the door, but cannot bring himself to get rid of any of his stuff. When he was 12, he began collecting baseball cards, then books, and then magazines. His parents were poor immigrants who instilled the value of keeping things that might be valuable someday. However, after a while, his "collection" made their family apartment so cluttered that they threw much of it away. He retrieved it from the trash and from then on his "collecting" has been a source of conflict between himself and his parents and future employers. The thought of throwing any of these things away makes him extremely anxious, because "there might be something of value written on it." It has cause conflicts with employers when he has brought stuff to his workplace to "store" and even lead to fist fights with maintenance workers when they tried to throw out some of his things. He was married for 16 years, but it ended in divorce after his wife could no longer tolerate his behavior. His own apartment has not gotten filled up and he injured himself trying to move things so he could walk through his apartment. He abandoned his apartment and is currently sleeping on a cot in the basement. Even though he knows it is irrational, he can't bring himself to throw any of his stuff away.
What to do
1. Read the case study and identify the symptoms presented
2. Select 3 Disorders that at least partially fit this description and identify why they might be plausible
3. Rule out 2 of the diagnoses and why they don't fit as well as the third (this can include things such as prevalence rates, age of onset, etc)
4. Present the most likely diagnosis and why you think it is the most likely (compare symptoms, consider demographics and statistics about the disorder) and include one question (or more) that you would want to ask to solidify your diagnosis.
5. What treatment recommendations do you have based on this diagnosis?
