What has changed in the industry to reduce medical errors since the To Err is Human: Building a Safer Health System was published in 1999?
Since To Err is Human, was published in 1999, a vast reduction in medical errors across healthcare facilities and organizations have been seen. Organization have made strides in technology and procedures that help monitor and prevent medical errors. Not only has technology help pave the way to reduce these medical errors, but more healthcare organizations have also taken an approach at better utilizing human errors and learning from them to strengthen their policies and procedures.
Do you think there is a disconnect between health care policies, protocols, procedures, or processes?
I believe that anytime people play a vital role in policies, protocols, procedures or process a certain amount of disconnect can be expected. People are mostly emotional, and no amount of intellect will pause a person raw emotion in the heat of a scenario or situation. For example, as many of the healthcare organization still have a foundation based on centuries of policies already residing in place. When new physicians are in their last years of schooling or practicums, they are being educated in the real world with those centuries old policies and procedures however are learning of newer ways to approach scenarios. When they receive their degree and start out in the workforce, they are already bringing a since of disconnect or disarray.
What are two solutions or recommendations to minimize medical errors in the health care industry?
Two solutions that could minimize medical errors could be, the implementation of the six rights. Right patient, right medication, right dose, right time, right route, and right documentation. In doing this, you can provide a more positive atmosphere for the staff as well as ensuring the highest quality of care to the patients. Another solution could be the implementation of a more in-depth monitoring system. Instead of what most EMR systems have today of have what is called “soft stops”, places in the system that allow for an override when the physician doesn’t have the order in the system and provides a verbal order, they staff would have to have an order in place prior to removing medication out of the system and administering it to the patient. Doing this would put a safer procedure in place to ensure the medication taken out will follow the six rights solution.
