The facility has three wings – Tamar,Derwent, and Gordon. After seven months of fulltime work, you feelcomfortable with the routines and culture of the facility. Todayyou are working the afternoon shift (1500-2300 hours). Your shiftteam includes: Claire – Facility Manager Registered Nurse (RN) andNurse Practitioner (NP) Claire is the manager of the facility.Claire is an experienced manager and clinician. She finishes at1730 hours. Rory – Clinical Coordinator (CC) Registered Nurse (RN)Rory is the Clinical Coordinator. He has worked in the aged caresector for the past ten years in a variety of roles. He has apassion for wound care and assessment. Rory’s rostered shiftfinishes at 1600 hours. Binsa – Enrolled Nurse (EN) Binsa is anEnrolled Nurse; she has worked at Broken River for the past 6years. Today Binsa is working 1600 – 2100 hours. Joshua – EnrolledNurse (EN) Joshua completed his Diploma of Nursing two years ago.He works with a nurse agency – frequently working in the aged caresector. He has completed several shifts at Broken River. Joshua isworking 1500-2300. Anna – Extended Care Assistant (ECA) Anna is anECA. She has completed a Certificate III in Aged Care. She hasworked at Broken River for the past 4 years. Anna is working1600-2100. Heena - Extended Care Assistant (ECA) Heena is an ECA.She has completed a Certificate III in Aged Care; she has worked atBroken River for the past year. Heena is also in the first semesterof her Bachelor of Nursing. CASE STUDY NUR350 ASSESSMENT TASK 2Lisa - Extended Care Assistant (ECA) Lisa is an ECA. She recentlycompleted her Certificate IV in Aged Care. She has been employed ona causal basis for the past four weeks. Max - Extended CareAssistant (ECA) Max has worked at Broken River for the 22 years. Inaddition to working as an ECA, he also works shifts in ‘leisure andlifestyle’. He knows all the staff, residents, and their families.Max is known for his cheerful approach to work. Caitlyn – NursingStudent (BN) Caitlyn is a student on the second placement of herdegree. You are Caitlyn’s preceptor today. Caitlyn is anenthusiastic student, who asks insightful questions. Handover Atthe commencement of your shift, handover is completed, you takeparticular note of: • Mrs June Jenkins – June is 77 years of age;she has lived at Broken River for the past 3 years. June isreceiving end of life care; her family are with her, including herhusband, George. Mrs Jenkins has Alzheimer’s disease. She has alarge supportive family; however, they have not accepted herdementia as a life limiting illness (despite education and referralto support services), and frequently in the past have asked staff‘why is she not getting better?’. At the completion of handover,you assess Mrs Jenkins, and check the syringe driver. You talk withthe family about her care and what to expect. They are distressedbut happy with the care being provided. • Mr Bob Archer – Bob is 82years of age. He owned his own plumbing business and was an activecommunity member. Bob’s wife died at Broken River three months agoand staff have noted his increasing frailty in this time. He enjoyswatching the Football on TV, country music and a glass of red winewith his evening meal. He has a supportive daughter, Carol who isan RN at a local private hospital. Carol visits 2-3 times per week.Bob has a history of hypertension, benign prostate enlargement,depression, and gout. He is a tall man (188cm) and has a BMI of 28.At handover staff report Bob has been a little teary today and doesnot seem himself.
Situation At 1745 hours, while residents are returning from thedining room to their rooms – many with the assistance of the ECA’s,the following occurs • Lisa comes to in distressed state to informher that Mr Archer has fallen in his bathroom (between the wall andthe toilet) and ‘there is blood everywhere’. • Max comes to tellyou that Mrs Jenkins has died, and the family don’t know what todo.
