You are an RN working in the Emergency Room, and at 10 pm, an
ambulance arrives with a 34 year old woman (Miss A) who is gasping
for air and says she is unable to breathe. She believes she
is having a heart attack because she also has chest pain, and keeps
saying she as chest pain, nausea, palpitations, dizziness, and
feeling of unreality. She is focused on the breathing and
chest pain, and keeps saying she is going to die and that she is
not going to make it. She is desperate, and keeps breathing
rapidly and hard and is diaphoretic.
Her vital signs are:
BP: 174/96
HR: 105
RR: 28
T: 98.6
Miss A was at work and was going to present at a meeting that
she had prepared for and would be important in getting a
promotion. She started having difficulties and these became
so bad, a coworker called 911 to have her seen at the
ER.
Miss A had a cardiac workup, and is still in the ER. After
blood work and EKG are reviewed, the doctor found no cardiac
problems and has determined that she has had a panic attack.
The doctor directs you, the nurse, to give Miss A Lorazepam 1 mg IV
push.
Miss A asks you why she felt she could not breathe.
Miss A also asks if there are some things that she should do to
avoid having these in the future.
