Mr. K's employer drives him to the emergency department after an accident on the factory floor. The employer tells the admissions nurse: "Listen, I suspect the guy is suicidal and that this was no accident. I have all the sympathy in the world for him-his wife left him-but I need to know if you think he is suicidal or did this on purpose to injure himself." Mr. C is a senior in high school who has attempted this evening to kill himself. As he is being admitted, his parents beg the staff to please keep this completely out of his medical records, as he is the recipient of numerous college scholarships and they are afraid this will hurt his chances of still being accepted in the fall and retaining his scholarships.
