Patient is in moderate respiratory distress, with audible
wheezes heard from the doorway. He has accessory muscle use with
intercostal retractions and is tachypneic. Head is atraumatic, eyes
have no icterus and mucus membranes are moist. Ears have clear
tympanic membranes. Heart is tachycardic at approximately 120 bpm,
but rhythm is regular. No murmurs, rubs, or gallops. Extremities
and back are nontender. Abdomen was soft, nontender, and had no
distension. No rebound or guarding. Neck was supple with normal
range of motion.
