Zoran is a 19-year-old, full time university student. He keeps
fit by riding mountain bikes, running and plays with the university
hockey team. He lives in a share house while on campus and goes
home for holidays. He also works part-time as a bartender. Earlier
today he went to the beach with some friends and hit his head on a
sand bank when diving into the water. He was floating face down in
the water for about 30 seconds before his friends realised that he
wasn’t fooling around. His friends alerted the lifeguards who
arrived very quickly. The lifeguards retrieved Zoran from the ocean
using a spinal board and maintaining spinal precautions as best as
they could. The paramedics were called who arrived 15 minutes
later.
You are working in the ambulance bay at the local hospital and
receive a quick handover from the paramedics. They explain to you
that Zoran is unable to move his arms and legs and has no
sensation. Although he hit his head there is no indication that he
has a head injury. The last set of vital signs taken by the
paramedics were: BP 100/72 mmHg, HR 82 bpm, RR 22 bpm (shallow),
SpO2: 92% on O2 15L/min via non re-breather mask, temp 35.2. They
have administered 250mL of normal saline & 4mg IV Ondansetron.
The paramedics report that Zoran has some pain to his cervical
spine. You are assigned to care for him. You are about to walk into
his bedspace when you overhear Zoran talking to his mother who has
recently arrived at the hospital. He is noticeably upset and says
‘Mumma I’m scared’.
