Make sure to consider Maslow's hierarchy of needs, threat to life, and client preference when you write your rationale. Mr. Smith is a 50-year-old man who was admitted with low back pain which he sustained as a result of heavy lifting at work. He is currently on bed rest with bathroom privileges and is in traction when in bed. He describes his lower back pain as being "very severe" and requests Oxycodone, a narcotic analgesic, every three hours. The doctor has ordered the medication to be administered every four hours as needed for pain (PRN). He is also taking a muscle relaxant medication and a stool softener. He ambulates to the bathroom under the nurse's supervision because, at times, he is unsteady on his feet.
