My 58 year old white male visits with an intermittent cough for 4 days. The cough was dry for 2 days but now he's coughing up thick pale yellow phlegm. He is smoking less because smoking seems to make the cough worse. He feels very tired and thinks he has a fever. Tylenol helps a little when taken. About 1 1/2 weeks ago was playing cards with friends and one of them was sick. Him and his wife deny any confusion. H/O Hypertension and DM type 2, 40 pack per year smoker. he's ran out of lisinopril 20 mg daily and metformin 500 mg twice daily meds because he hasn't seen provider in 7 months. Allergies: Penicillin. No alcohol/drugs. Ht. 5'4", wt. 190 lbs, BP: 150/90, pulse 88, resp. 26, temp. 101.0 oral. What history should I obtain. What is a Primary diagnosis and 3 differential diagnoses? What physical exam should I obtain? What labs and diagnostics to order? BUN is 21, what is his CURB score? Chest x/ray show infiltrates, how to treat?
