Case Study, Chapter 76, Disorders in Fluid and Electrolyte Balance A 62-year-old male, James Howard, is hospitalized with dyspnea and orthopnea. His medical diagnoses include chronic renal failure, poorly controlled type 2 diabetes mellitus and hypertension. He reports a recent weight gain of approximately 10 lb and profound swelling of his lower legs bilaterally. When the nurse examines him, he notes crackles in the lungs bilaterally and 4+ pitting edema distal to the knees bilaterally.
