Case Study, Chapter 76, Disorders in Fluid and Electrolyte Balance A 62-year-old male, James Howard, is hospitalized with dyspnea and orthopnea. His medical diagnoses include chronic renal failure, poorly controlled type 2 diabetes mellitus and hypertension. He reports a recent weight gain of approximately 10 lb and profound swelling of his lower legs bilaterally. When the nurse examines him, he notes crackles in the lungs bilaterally and 4+ pitting edema distal to the knees bilaterally. James' lab work indicates a potassium level of 5.5 mEq/L and an ABG result with abnormally low pH and HCO3 readings and a normal CO2 (Learning Objectives 3, 4, 6, 7) 1.
