Mr. Jones has multiple prescriptions of Coumadin in his home and
he is unclear as to which ones he is supposed to take. He has been
taking Coumadin without questioning anything. His Primary diagnosis
is Chronic Atrial fibrillation; Dr. No has prescribed different
dosages of Coumadin .Dr. No prescribed Coumadin for 5 days in
tapering doses. He is discharged today August 20, 2020.
Take Coumadin 20 mg. PO today August 20, 2020
Coumadin 15 mg. PO August 21, 2020
Coumadin 10 mg. PO August 22, 2020
Coumadin 5 mg. PO August 23, 2020
Coumadin 2 mg PO August 24, 2020
Have a blood draw for PT on August 25, 2020
Call Dr. No if you have any problems with bleeding. Refrain from
eating green leafy vegetables while you are on Coumadin.
As a Charge Nurse communicate with the Home Health Nurse using
SBAR.
