Yousuf is a 13-year-old boy and a known asthma patient. He is admitted to the short-stay unit following complaints of persistent dry cough, runny nose and difficulty in breathing for the past two days. He looks tired and speaks in very short sentences. He has increased work of breathing with obvious nasal flaring, biphasic stridor, and he is sitting on the edge of the bed and leaning forward, which he finds more comfortable to breathe. His observations are as follows: Temp 37.9oC, oxygen saturation =90%, respiration rate=38b/m, heart rate 125 b/m, capillary refill of 2 seconds, BP 140/65. He is alert with no pain. He has been prescribed nebulised salbutamol, 4L oxygen therapy via mask and intravenous hydrocortisone.
