The admin nurse will collect general information about the presented person including name, date of birth, address, and Medicare. A patient is then seen by a triage nurse who will determine priority (urgency) level (1-5) and collect some medical history data including chronic diseases and current medications, then the nurse would collect the reason for admission to ED and current symptoms.
