Bruce is a single, 38-year-old man who is overweight (BMI 28) and has recently been diagnosed as pre-diabetic. He is an engineer by profession and before COVID, he had a daily routine stopping at the gym and working out for an hour before going home after work. But during the worst of COVID, the gym was closed and he was forced to start working from home and it changed all of his habits. He started moving less, eating more, and even socializing less. Now he's back at work, but can't seem to get into his old routine again, and he now has this new diagnosis. Bruce does want to avoid having the pre-diabetes progress to diabetes and wants to avoid any medication, so he comes in to see you, a nutrition counselor, because he wants your help. This is the 2nd visit with you. You had asked him to fill out a log of cognitions form after the first visit, at least for one day. He brought this back to you at the 2nd session:
Date May 10 Log of Cognitions Time Place Food Eaten Thoughts about Eating, Thoughts About Eating Before/During Eating After Eating 7:30 Bowl of Corn This is kind of boring. Home Quick and easy Flakes and tastes like AM cardboard, but it's better than really sugary cereal that I used to eat 7:30 Home Cup of instant I am really tired and Instant pick-me-up AM coffee need the caffeine 12 noon Mario's Pizza Pizza - 2 slices It's nearby my office And boy was it good and I have missed good pizza since working from home 12 noon Mario's Pizza Diet Pepsi I'm not getting the It doesn't taste the regular Pepsi same as the regular, but It was still good 3:30 PM Work- 2 small chocolate This is my time to Really good - and they employee's chip cookies socialize a little with my are small lounge colleagues 3:30 PM Work - Cup of brewed This is my time to Much better coffee employee's coffee socialize a little with my than the instant I make lounge colleagues at home 6 PM Home Bowl of pasta with Easy to make the I was so annoyed with tomato sauce tomato sauce is myself for letting the vegetables. But when I pasta stick to the pot I boiled the pasta. I didn't enjoy eating it
6:30 Home Canned peaches forgot about it and a lot and I knew I would need of pasta stuck to the to scrape that pot pot I know I should have I love peaches and some fruit every day there aren't any fresh ones this time of year I was hungry again while It's salt and not sugar, watching Netflix so that's good. 10 PM Home Tortilla chips .) How would you begin the second session with Bruce? 2) What stage of change do you think Bruce is in? 3) What will you put on his PES form as his nutritional diagnoses? 4) What questions would you want to ask Bruce to follow up and discover more? Come up with at least 3 questions. Make sure your questions are open-ended. Make up some answers he might reply to your questions. 5) Do you think Bruce might have an issue with self efficacy? Explain. 6) What might be some barriers for Bruce? 7) What do you think might be motivators you could emphasize with him? 8) Come up with 3 suggestions for Bruce, so he can choose a goal to strive for until you see him again in 1 week. (It would be preferable if Bruce could come up with his own suggestions, but in case he can't, you can give him some possibilities and let him choose). 9) Once he has chosen a goal (for purposes of this quiz, choose one of the 3 goals for him, although in a real counseling session, he would choose his own goal). Make it into a SMART goal – Specific, Measurable, Attainable, Rewarding, Time-bound
