You are caring for a 50-year-old male patient recently diagnosed with Type II Diabetes when he presented to the Emergency Room with a wound on his plantar area of the left foot, that “started as a blister from his shoes rubbing” and then according to the patient, “it got bigger and bigger and then it opened and started draining”. Pt states that he tried to “soak it Epsom Salt baths” as recommended from his neighbor. Then, his sister had a few antibiotic pills left from an infection she had, so he took a few but it “doesn’t seem to have helped”. He said that it started to “drain a lot of yellow stuff and bleed”. He admits that his doctor diagnosed him about 5 years ago with HTN and “high cholesterol”. He said that it is hard for him to always have the money for his medications. He is in Construction and states he has been “in between jobs” for a while and has no health care coverage. Patient states he is trying to just “eat better” and “cut down his smoking” so that his blood pressure and cholesterol can get better.
Pt’s serum glucose on admission was 390 and the HGA1C: 12.
He admits to the hospital with a severe diabetic ulcer on the plantar of the left foot. The pt is pending a MRI / MRA with possible contrast to determine a diagnosis of Osteomyelitis .
