Missy Wike is a 28 year old female admitted to the YTI Hospital last night for weakness and what appears to be ascending muscle paralysis. She is on the Med/Surg floor, she is alert and oriented. Chief Complaint: Weakness, difficulty walking Subjective Data: Patient reports h shaving an upper respiratory infection 2 weeks prior to coming to the hospital. Family doctor recommended over the counter medications and rest. Patient states that she was feeling better but that she had "pins and needles" in her feet and legs starting 2 days ago, much worse at night. She reports leg weakness and difficulty walking. Objective: Awake, alert, oriented young woman; no significant past medical history and no contributing family history. Patient does not drink alcohol, use tobacco products or vape, or use illicit drugs. Physical findings: Heart rate 96 . Blood pressure - 134/82 . Temp - 37C . Respiratory rate - 24 and shallow Breath sounds decreased throughout . Chart vasar
