TJ has end stage lung cancer and with pain in his chest and
spine. He is taking oxycodone sustained release (SR) 40mg q12h and
had been using approximately 2 doses of 10mg oxycodone immediate
release (IR) a day for breakthrough (BT) pain. He states that
he was fairly comfortable however, he is now experiencing shooting
pain down his legs. The prescriber is willing to change to
methadone if you will help him dose him and monitor him. What
are your recommendations?
