The pain was constant, sharp, and radiated to the left arm. On examination, it was noted that there were small red maculopapular lesions, like insect bites, that he attributed to a recent contact with shrubbery during his hiking trip 2 days earlier. He is admitted to the medical unit for evaluation of acute coronary syndrome. Sasha EKG's remained normal, as did serial troponins. The intense pain in the left lower chest continued, with radiation now to the back. Two days following Sasha's admission, you noted a new onset of a vesicular rash on the left chest. Over the next day, it progressed to a painful, pruritic rash. On physical exam, you note there were no other abnormalities, and his lung examination was normal. You verify his blood work and note that his blood white cell count and neutrophils were normal. Remaining laboratory studies were normal.
