You are a registered nurse at an endocrinologist office and you
are about to enter the room of your next patient, Robert (Bobby)
Thompson. Bobby is a 32-year-old caucasion male who has diabetes
mellitus type 1. When reading his chart, you learn that Bobby was
diagnosed with diabetes in his early 20s after experiencing blurry
vision, polydipsia, and polyuria while at work. You also note that
his diabetes is uncontrolled with frequent hospitalizations. When
you enter the room you see Bobby accompanied by his mother. Bobby
is pleasant but appears to be a little frustrated. He stated that
he was worried he would not make his appointment on time because
his car broke down again this morning and he had to have his mother
drive him. When Bobby leaves the room to use the restroom, his
mother speaks up about concerns she has. She states that she is
worried about Bobby. He has made comments to his siblings that he
will never live to see 40. In fact, recently he purchased a bottle
of sleeping pills and when he got into the car with his sister, he
stated that he could just take the whole bottle and would not be
here tomorrow. The sister’s husband then asked to see the bottle
and threw it out the window. Some days he is very happy and other
days he seems really down. He is recently divorced and does not get
to see his 6-year-old son as often as he would like mainly due to
transportation. His car continues to break down and every time he
fixes it, something else stops working. He currently needs a
battery and his siblings are getting frustrated with having to jump
his battery or having to give him a ride all the time. His mother
states that he is currently living with his father because his
diabetes is uncontrolled; everytime he gets a new job, he is
hospitalized with his diabetes. The mother went on to state that
she met with Bobby’s siblings and told them they better do
something or he will not be here much longer. Bobby’s siblings said
that he is just talking and that he won’t do anything. They believe
that everyone has hard times and Bobby needs to toughen up. She
goes on to say that maybe he is really fine because yesterday he
was very happy at the Super Bowl party, especially when his team
won.
