George is a 17 year old male who has Muscular Dystrophy. He is admitted because his parents state that he has had a cold
questions due to his shortness of breath. You note that he seems to be anxious and restless-almost panicked. His vital signs are 130/86;T-98.9, P-104; R-26. No cough is noted, but you do note that he has a lot of phlegm in his mouth. Pick out the assessment data and determine the priority nursing diagnosis. George is a 17 year old male who has Muscular Dystrophy. He is admitted because his parents state that he has had a cold and it has become increasingly difficult for him to breathe. He seems to have excessive mucous as you hear crackles throughout his lung fields. He is having trouble answering
