George is a 17 year old male who has Muscular Dystrophy. He is admitted because his parents state that he has had a cold and it has become increasingly difficult for him to breathe. He seems to have excessive mucous as you hear crackles throughout his lung fields. He is having trouble answering questions due to his shortness of breath. You note that he seems to be anxious and restless-almost panicked. His vital signs are 130/86;T-98.9, P-104; R-26. No cough is noted, but you do note that he has a lot of phlegm in his mouth. Pick out the assessment data and determine the priority nursing diagnosis. (See note above regarding the list) Plan-what is an appropriate patient centered goal for your patient? Be SMART about it. Implement-What interventions will we do to help the patient meet the goal? List at least 4.
