primary care clinic for continued back and shoulder pain x 3
weeks. EF is a truck driver and loads/unloads trucks on a daily
basis and delivers nationally often. Patient currently
is uninsured

PMH: HTN, T2DM, Scoliosis, migraine, angioedema
