A 50-year-old woman checked in at the emergency department in a Maryland hospital in late August complaining of a high fever with chills and headaches for the past five days. She also mentioned a decrease in appetite, nausea with vomiting, and diarrhea. Her temperature was 104°F and scleral icterus was noted. Laboratory findings suggested a possible hemolytic disease, and elevated liver enzymes and serum bilirubin were also found. When questioned about her recent travel history, she reported a trip to Spain and Portugal five months earlier. She acknowledged to live in a rural area of the northeastern United States and mentioned that her husband was diagnosed with Lyme disease one year earlier.
