You the student nurse have been assigned to 58-year-old women, post-operative day number one following a total abdominal hysterectomy. Your instructor tells you this patient has been struggling during the night with pain management. Morphine sulfate is one of the drugs used in her current care. As a result of this upcoming patient care assignment the pre-conference discussion that morning focuses on pain management for those with both acute pain and chronic pain. Answer the following questions as they relate to acute pain. 1. Explain the role of the 3 main classes of opioid receptors. Include a discussion of each receptors response when activated. 2. Distinguish among the classification of drugs that act at opioid receptors (pure opioid agonists, agonist-antagonists, and antagonists). Be sure to give a prototype drug example for each classification of drug. You medicate your patient with 4 mg of morphine IV push. You return to your patient 10 minutes after administering the morphine. You find her respiratory rate is 8 breaths/minute, her oxygen saturation is 87% on 2 liters of oxygen and she is difficult to arouse. 3. Discuss the classification of drug you anticipate will be used to reverse this patient situation. Include a discussion of the action and therapeutic use of this drug. Following the above event your patient discusses her concern about the possibility of becoming addicted to the morphine. 4. How would you proceed in addressing their concerns of addiction and opioids? 5. How would you explain physical dependence on opioids? 6. How is abstinence syndrome prevented when patients are physically dependent on opioids?
