Fanny Mace, a 63-year-old female client, was admitted to the
hospital with sepsis 3 days ago and is being treated with
vancomycin IVPB. The client has a history of heart failure and
takes furosemide (Lasix), potassium chloride (K-Dur), and
irbesartan (Avapro). The nursing assistant (NA) received complaints
from the roommate in the semiprivate room that the client’s
television is too loud. The nursing assistant is in front of a
sunny window and tells the client to turn down her television. The
client leans forward, cups her hand over her ear,stares at the
nursing assistant’s lips, and asks her to repeat the information.
The client still does not understand. The LPN/LVN had entered the
room to assess the client and saw what happened and needs to
provide care for the client with the hearing loss.
