RECENT EVENTS A 77-year-old female was brought to the emergency room after falling twice at home. The first fall was in the bathroom, slipping while she was getting out of the shower. She fell a second time, 2 hours later, when leaving the house to visit a friend. Her daughter, who was present during both falls, states she hit her head on the door frame while exiting the house. There was a large hematoma on the right side of her skull. She has been admitted to the hospital at this time, where, hopefully she will regain her strength and be able to return home. RECENT HISTORY The daughter states that her mother has been fatigued over the past 3 weeks and began taking an herbal remedy suggested by a friend. The daughter also states her mother had complained about her arthritis giving her problems. When further information is sought from the daughter, she states that she added garlic capsules to her mother's daily medication regimen in order to lower her cholesterol levels. The garlic was not on the list of home meds provided. Also, as the daughter pulled the medication list from her mother's purse, a box of Goody's Headache powders fell out. HOME MEDICATIONS Aspirin 325 mg oral daily Furosemide (Lasix) 40 mg oral daily HCTZ 25 mg oral daily Metoprolol succinate (Toprol XL) 50 mg oral daily Potassium (K-Dur) 20 mEq oral daily Rosuvastatin calcium (Crestor) 10 mg oral daily Warfarin (Coumadin) 5 mg oral daily 5 Clopidogrel (Plavix) 75mg daily Cyproheptadine 4 mg PO daily Gabapentin 1200 mg tid for nerve pain Neurotonin 600 mg at HS for restless leg syndrome Now added: Garlic 2 capsules every morning Unknown Herbal remedy patient took for fatigue ALLERGIES: Codeine (facial swelling)
