Endorsing to you patient PBG, 32 years old male, single
with chief complains of recurring epigastric pain, 6-7/10 in
severity, loss of appetite, episodes of diarrhea and a feeling of
fullness in upper abdomen after eating. Patient is admitted at
Sam today under the service of Dr. P. addaw with admitting
diagnosis of Peptic Ulcer Disease. The following are admitting
orders:
Admit the patient to room of choice under my service
Secure consent prior to admission
Hook patient to D5LRS 1L x 8h
Monitor vital signs q4 hours and record.
Monitor I and O per shift and record.
Prepare patient for gastroscopy tomorrow morning
Start Amoxicillin 500 mg per cap 2 caps PO BID
Clarithromycin 500 mg per tab 1 tab PO OD
Omepron 40 mg IV every 12 hours
Prevacid FDT 30 mg per tab 1 tab PO BID ac
Zinnat 500mg 1 tab PO BID
Mucosta 100 mg 1 tab PO TID ac
Ketorolac 30mg IV PRN for gastric pain
Refer for untoward signs and symptoms
