A 50-year-old woman has been experiencing a chronic cough forthe past two months. She is a two pack a day smoker and has beenfor the past 30 years. She sees her Primary Care Physician (PCP)for the cough and is given an order for a chest x-ray at the localhospital. The x-ray report states “nodule in lower lobe of rightlung, worrisome for malignancy.” The PCP refers her for a biopsy ofthe nodule at the ambulatory surgery clinic. The ASC sends thebiopsy to the Pathologist who substantiates the diagnosis of cancerin the pathology report. Two weeks later, the same physician whodid the biopsy performs a right lower lobectomy. Prior to dischargeafter surgery, the patient develops an infection and the infectiousdisease specialist is asked for a consultation to evaluate her andrecommend the appropriate treatment course. Upon discharge, shereceives care in her home from a Home Healthcare Agency for thenext two weeks. Four weeks after surgery, radiation therapy isinitiated, but her condition continues to deteriorate over the nextfew months with metastasis to the brain noted, at which point shedecides to receive only palliative care in a hospice facility.
