Dalton is a 14-year-old male client who was involved in a motor
vehicular collision with a tree. The dirt bike that Dalton was
riding fell on him, burning his lower left leg and thigh.
Dalton is diagnosed with a deep partial-thickness wound of the left
lower extremity. The emergency room physician orders the nurse to
clean the wounds to prevent Dalton from developing sepsis. Dalton’s
mother becomes visibly upset and states, “Sepsis, people die from
sepsis, how can this happen?”
Dalton received the tetanus-diphtheria (Td) vaccine when he was
initially admitted with the burn injury. Dalton’s mother states
that he is up to date with all of his vaccines.
Dalton also received amoxicillin as prophylactic treatment
during the initial phase of the burn injury. Within a few hours of
receiving the amoxicillin, Dalton developed chest tightness,
wheezing, and a red raised rash with pruritus.
Dalton is discharged from the acute care setting, and his mother
was taught how to perform his dressing changes to his leg. As the
nurse in the wound clinic, Dalton’s mother asks why there is so
much drainage from his leg?
As Dalton heals from his burn injury, he develops
recurrent infections. Dalton is told that he has an
immunodeficiency.
Dalton develops frequent pseudomonas aeruginosa
infections since his initial burn injury; however, it becomes
harder to treat with each infection. Dalton asks how this
happens?
Each time Dalton has an infection, he also has issues with
controlling his blood sugar levels. Dalton is told that he is at
risk for the development of diseases related to his chronic stress
because of the chronic inflammation with recurrent infections.
Dalton is concerned about developing cancer from chronic
inflammation.
Dalton asks if there are any dietary changes he could make to
protect him from developing cancer.
