Investigation must be prompt
All site activity must stop
All activity affecting the equipment must cease
The site and fall equipment is secured
Investigator must determine to resume activity/secure orrelease equipment
Competent person must inspect all equipment associated with thefall
Investigation must be completed and kept on file for at least 5years
