Mr. Devin Anders enters his Primary care Physician (PCP) for an interim office visit. He has Hypertension and Diabetes Type II. For the past few weeks his BP (blood pressure) has been extremely high. He has multiple symptoms today. His chief complaints are headaches, blurred vision, and leg swelling, His PCP has decided to change his BP medication as one of them was causing the leg swelling, also 2 new medications were added to help keep his BP under control. During the office visit a finger stick was done to check his BS (blood sugar), it was high >300. Mr. Anders has been compliant with his BS medications, but his BS are still running high. His PCP has also decided to start insulin, and he most likely will be on insulin for the rest of his life.
