Pathophysiology The thickening of the valve structures reduces the outflow of blood from the LA to the LV. The LA dilates to accommodate the amount of blood not ejected, and left atrial pressure increases. As left atrial pressure increases, the blood volume backs up into the pulmo- nary system, increasing pulmonary pressures. This
increases the workload on the right side of the heart, leading to right ventricular hypertrophy. Eventually, the RV fails, and cardiac output decreases because less blood is delivered to the LV.
