A 57 years old woman is looking for the doctor in
August 2019 with cough, fever and shortness of breath. She is
a non-smoker, normal weight, previously healthy and does not use
prescription medications.
she became ill about a week ago with mild respiratory symptoms
(feeling of discomfort in the throat, dry cough and fever). The
symptoms debuted 3-4 days after she had cared for one of her
grandchildren over the weekend. She first got better but last night
she deteriorated rapidly with fever, pain in the right chest and
cough with mucus that looks yellow. She has had difficulty sleeping
during the night due to the cough.
When she is examined at the health center, she is tired, short
of breath, coughs a lot and has a fever of 38.4 degrees but can
account for what has happened and does not look confused. She
breathes at a rate of 24 breaths / minute. Her blood pressure is
normal (140/80) but her heart rate is elevated (> 90).
The doctor examines her lungs with a stethoscope and seems to
hear a rattling lung sound. The breathing sounds are possibly a
little less clear at the bottom of the right lung.
