CASE STUDY:
You are the nurse-manager of a pediatric unit. A 13-year-old,
hispanic girl has been newly diagnosed with insulin-dependent
diabetes was recently admitted to your unit. Her glucose levels
have been difficult to control and the nursing staff have taken a
special interest in her case. The patient is very easy going and
cooperative, however, the family is loud and visits frequently in
large numbers without regard to visiting hours. The visiting family
are consistently bringing her large amounts of desserts and
sweets.
The nursing staff has come to you on three occasions and
complained about the family’s noncompliance with visiting hours,
gifts of unhealthy foods, and disturbing other patients with loud
conversations outside the room. You feel conflicted because
the nurses are aware of the culturally sensitive plan of care for
this patient but several hospital policies are being violated and
the nurses want you to solve the problem.
That same day several family members approached you and
complained about the patients strict diet, hospital visiting
policies, and the rudeness and disrespect of several of the
nursing staff and aids. After a long meeting with the family, they
seemed to understand and left happy.
The next day, one of the nursing staff told the family that
according to hospital policy, only two members could stay and if
the others did not leave, she would call hospital security. Later
that evening, another nurse accused a family member of causing harm
to the patient by providing excessive sweets. As a result of these
encounters, the parents asked the nurse to request a discharge from
the doctor. The patient’s diabetes remains unstable and the
diabetic educator has only visited with the parents twice. You feel
that it is too soon for the patient to be discharged.
