Mr.D is a 53-year-old patient on the medical-surgical unit for
the management of chronic bronchitis and pneumonia. He worked in a
paper mill factory, where he inhaled sawdust and chemicals daily.
Working in an industrial environment caused his lungs to be
constantly irritated and inflamed. You are the nursing student
assigned to Mr. D. After reviewing his care plan, the health care
provider’s orders, and the nursing notes from the previous shift,
you enter Mr. D's room. You find Mr. D restless, agitated, and
confused. His pulse is 106 beats/minute, and respirations are 39
breaths/minute and shallow. He is sitting up in bed grasping the
side rails and trying to catch his breath.
What steps will you take to assess Mr. D? Give the
rationale for your choices. using ippa
