Safety Scenario Mr. Smith is an 87-year-old male resident at your facility. Mr. Smith has the following history: . . CHF, DM type II, HTN, AKI, COPD, on home oxygen at 2L NC, Parkinson's Disease, Afib, CABG x2 in 2001, and a RTKA in 1998. Medications: Lasix, insulin, Lopressor, Eliquis, Sinemet. Uses a walker to ambulate due to gait issues from Parkinson's. Is care planned to use restroom by himself with walker and has concentrator in room. Able to ambulate in hallways with assistance due to tripping hazard. You enter Mr. Smith's room. You find him on the floor. He was attempting to use the restroom as he is care planned. . 1.
