A previously healthy 40 years old female suffered from double
vision (diplopia) after sustaining a minor head injury that
occurred when she lost control of her motorcycle on a curve.
Examination of her right eye showed ptosis, lateral deviation of
the eyeball, and a dilated, non-reactive pupil measuring 6 mm (see
Figure below). Ophthalmologic tests of vision and intraocular
pressure were within normal ranges. The left eye was normal. The
patient was diagnosed with the oculomotor nerve palsy.
Explain the clinical symptoms presented above from the
anatomical point of view:
• Lateral deviation of the eyeball (pupil is fully abducted and
depressed; ‘down & out’)
• Dilated non-reactive pupil • Ptosis (drop of the superior
eyelid)
• Discuss test for extra-ocular muscles
