Traditional Style FAIRPLAY MEMORIAL HOSPITAL Health Information Management Department Procedure: Admissions and Discharges General description: The purpose of this procedure is to maintain accurate records of patients' admissions to and discharges from the hospital. These records assist health information management personnel in locating both medical and demographic data on each patient. Also, admission/discharge/transfer records are essential to verifying the accuracy of statistical information prepared by the department for hospital administration. Tools Used in the Procedure: (1) Patient index - this index contains a 3 x 5 card for every patient who has been seen in the hospital since January 1, 1956. Patient cards are filed alphabetically. (2) In-house box - this is a small file containing 3 x 5 index cards of patients who are currently occupying a bed in the hospital. These cards are also filed alphabetically. Procedure: 1. At approximately 9 am, a clerk from the admissions department brings the following sets of forms to the admission/discharge clerk in the health information management department: 1.1 Hospital census sheets prepared at the previous midnight that indicate patients admitted and discharged the previous day. These sheets also show the names of patients who have been transferred from one floor or service to another. 1.2 Yellow sheets, each of which indicates one patient admitted to the hospital. 1.3 Blue sheets, each of which indicates one patient discharged from the hospital. 1.4 On Mondays you will receive the sheets from Friday, Saturday, and Sunday. 2. Cross checking hospital census sheets against the admission (yellow) sheets and the discharge (blue) sheets. 2.1 Check the hospital census sheets against the admission and discharge forms to see that there is a form for every patient listed. If a name appears on the census sheet but there is no. form for the patient, refer the problem to your immediate supervisor.
2.2 Check through the census sheets to determine if a patient has been transferred from one hospital service to another. If there is such a transfer, pull the patient's index card from the in-house file and make the change of service on the card. (Do not be concerned with patients who have transferred from one floor to another without changing hospital service). 2.3 After completing this check, place the census sheets in the messenger pick-up box. 3. Using the blue discharge sheets, pull the cards of all patients discharged from the in-house box. 3.1 Stamp the discharge date on each card. 3.2 If the patient has expired, stamp "expired" underneath the discharge date and under the unit number in the upper right corner of the card. 3.3 When the above steps are completed, give the discharge sheets to the discharge clerk. You will file the index cards of discharged patients into the patient index when you have completed the rest of this procedure. 4. Processing admissions. 4.1 Pull from the patient index the master card on every patient readmitted the previous day. Make sure that the hospital number and the name on the index card are EXACTLY the same as the number and name on the yellow admission forms. As you pull cards from the patient index, insert a pink card to replace the card you are pulling from the file - this marks the proper place for refilling and speeds up refilling the card at a later time. 4.2 If the address and phone number on the admission form and on the patient's index card are different, copy the current address and phone number from the readmission form onto the index card. 4.3 If you cannot find an index card for a given patient, check under other spellings, and under the maiden name. If this proves unfruitful, check the number index in the admitting office that lists in chronological order the hospital numbers and the names of the patients to whom numbers have been issued. 4.4 If a patient is admitted to the hospital for the first time, there will be no card in the patient index. Prepare the patient's card, typing in all the information required. 4.5 After all the patient index cards are pulled or made up, type the admission date, the ward, and the service on each card.
