Severe hemorrhage is classified as the loss of at least 1L of
blood. This leads to a condition called hypovolemic shock, whereby
blood volume is too low to meet the body’s metabolic demands.
Severe hypovolemic shock can lead to damage of the proximal
convoluted tubule and the thick ascending limb of the loop of
Henle, because they are the most metabolically active regions of
the nephron. This inhibits their ability to reabsorb solutes,
including urea.
Describe how this reduction in solute reabsorption would impact
solute and water excretion and explain the relevant mechanisms. Top
tip: Patients with this condition experience polyuria (excessive
urination) and need to be pumped full of fluids constantly.
