Code the following operative report
using ICD-IO-CM and ICD-IO-PCS
Preoperative
Diagnosis: Left breast mammographic abnormality
Postoperative
Diagnosis: Fibrocystic disease of the left breast
Operation: Left
breast biopsy with needle localization
Estimated Blood Loss: Minimal
Complications: None
Procedure: The
patient was taken to the operating room and placed in the supine
position and after induction of endotracheal anesthesia, the left
breast was prepped using Betadine solution and draped in a
sterile fashion.
A transverse incision was made in the
left breast medial to the insertion point of the needle. Incision
was taken to the subcutaneous layers and the tissue surrounding the
guidewire was excised using electrocautery. The amount of tissue
excised measured approximately 3 x 2 cm and was submitted for
specimen radiography. This was performed and demonstrated a portion
of the nodule in question to be in specimen and this was further
submitted for a frozen section diagnosis.
With this in mind, the deeper layers
of the incision were closed with interrupted 000 Vicryl. The skin
was closed with subcuticular 0000 Vicryl and a sterile dressing
was applied.
The patient was taken to PACU in good
condition after having tolerated this procedure well.
